Cosmetic Dental Bonding describes the procedure where various dental tooth-coloured restorations are glued to tooth substance, improving the seal and retention of a number of Dental materials, especially useful for cosmetic cases. We would recommend that ANY Whitening or Bleaching procedures are done first, so that any "NEW" bonding work can be done to match the new whiter shade of your bleached teeth (see pictures below).
Bonding enables your dentist to reduce spaces between your teeth, lengthen small or mis-shaped teeth or whiten stained or darkened teeth by bonding tooth-coloured materials matched to your own tooth shade. A single tooth can be shaped and coloured, or your entire smile can be changed using the latest cosmetic dental materials and dental bonding agents.
The Pictures below show how "bonding" can restore the looks and function of teeth, without drilling!

When teeth are chipped or beginning to wear, bonding can protect them from damage and restore their original look. Bonding your teeth with resin can be a one-appointment procedure, eg: First, teeth are slightly roughened and a conditioning gel is applied. Then a bonding resin is placed on the teeth (enamel/dentine) and is crafted to the desired shape. A high-intensity blue light then hardens the resin, bonding it to the teeth. Once in place, bonded restorations are very strong. Bonding can greatly improve the appearance of your teeth, resulting in a more beautiful, natural-looking smile! However, strong or worn bites may be unsuitable for this one visit procedure.
There are some other systems too that use bonding technology, but these will usually need 2 or more visits and will use various laboratory ceramics and porcelains. This increases the treatment time and the price, but may give a longer lasting improvement than bonded resins alone. Who should (and should not) perform tooth whitening?
WARNING: If tooth whitening is done by a Dental Care Professional (Dental Nurse/Hygienist/Technician etc.) who has not yet received competency training and certification, it could harm you, make your smile worse, or clash cosmetically with other dental work.
The GDC is very clear about additional duties for DCPs – they need to be competent and able to prove their competency to do even the simplest tooth whitening options.

Whitening as part of a full Smile Makeover
Your smile may need a combination of a number of dental treatments planned together, to maximise the effect of the tooth whitening or tooth bleaching treatments you desire. Tooth whitening alone cannot address everything.
For example, look at Dr. Kilcoyne’s results when cleverly combining tooth whitening expertise with special bonding techniques using cosmetic fillings, to improve the smile with minimal or no drilling, completed in only 2 days – a simpler case, but done well and within the patient’s budget where speed of result was needed.
Whitening is often done in combination with other dental treatments, so it is important to get an overall treatment plan so that the right things can be done to enhance the final result.
A very common oversight is to have a front tooth crowned/filled and then only consider tooth whitening a few months later. Unfortunately, only natural tooth substance will lighten with teeth whitening – crowns, veneers, fillings, denture teeth etc. all have chemical colouring embedded so don’t whiten with the natural teeth. We often find adults who have had our FastBraces (which can straighten teeth in months, not years like most traditional braces) like to get their teeth whiter once straighter. This can be done after as little as 2 weeks after brace bracket removal.

While new law changes have put off some salon-type businesses from experimenting on the public, we know there are some who will still try to act illegally and dangerously for profit, risking public health at an individual and community level.
They don’t have to have Hepatitis B vaccinations, follow HTM01-05 hospital-level cleanliness or the many other high standards that dentists meet routinely. They are not properly regulated or inspected and can disappear without trace as soon as problems arise, which may only show up with blood tests later (for example, liver failure).
There has been at least one legal case of a non-dental professional making a patient’s asthma permanently worse by using whitening chemicals – they were sued for a large amount of money. Many other legal cases are waiting to sue for irreversible damage. Insurance companies often do not insure such dangerous or illegal activity.
The General Dental Council (GDC) states that only registered and properly trained dental professionals are legal, trained and safe to take account of your medical history, use whitening systems safely and provide proper cross-infection and sterilisation controls.

Strangely, some members of the public don’t always report such salons or amateurs for failed whitening or possible disease transmission (AIDS, Hepatitis) – perhaps because they feel foolish or embarrassed having fallen for the “cheap” promises.
The same applies to internet products for tooth whitening – you simply don’t know what you are getting, whether it is sterile, contains waste chemicals or just harmful bleach or other poisons.

Over-the-counter “whitening” vs professional tooth bleaching
Why does tooth whitening seem to be claimed everywhere?
Tooth whitening refers to any form of tooth colour improvement and has been seized upon by various manufacturers – including toothpaste
companies – to imply they actively whiten teeth significantly, when in reality they mainly reduce surface stains.
Over-the-counter products are much weaker than professional systems for safety reasons, so their advertising and packaging can be misleading. Over-promises appear in the headlines, with disclaimers in hard-to-find small print.
Once professional whitening procedures have been completed, these over-the-counter “whitening” products can help maintain improvements for longer by reducing staining. But there is a lot of marketing hype that needs to be put back into proportion.
Only dentist-prescribed tooth bleaching actually whitens tooth substance significantly and safely, using a high enough concentration of hydrogen peroxide in a neutralised form, applied carefully to avoid gums, old fillings and crowns, so as to prevent damage, sensitivity or nerve problems.
Do not use normal hydrogen peroxide or unknown chemical sources claiming to be tooth whitening gels in your mouth just because they are cheap – they could easily cause permanent damage to teeth, gums or worse.
Professional tooth whitening by a qualified dentist or suitably trained Dental Hygienist/Therapist is safe, can often avoid injections or tooth removal and has great social and psychological benefits when done well – avoiding the dental and health damage that non-dental professionals can cause.
Veneers, crowns, dentures – and why whitening often comes first
We usually advise trying tooth bleaching first, to avoid the need for drilling tooth away – or at least to minimise the amount of more complex cosmetic work needed. Tooth bleaching can reduce the perceived need for more invasive cosmetic dentistry and even some cosmetic facial surgery.
Please note – if you have had problems with tooth whitening from people or establishments not qualified as dentists, we may still be able to help you overcome severe sensitivity, damaged crowns/veneers, permanent gum damage, mouth ulcers or simply very disappointing or poor results.
Do not confuse professionally supervised tooth whitening by a qualified dentist, after a full examination, with the dabbling of hairdressers or ear-piercers who may do permanent damage with so-called “cheap” materials “just like the dentist uses”. Unlike hair, teeth do not grow back if ruined, and uncertified/online chemicals can also harm unborn babies or permanently worsen asthma.
Some problems can only be detected by a thorough examination by a dentist – including cross-infection risks. It doesn’t take five years at university to qualify as a dentist for nothing.
Other tooth whitening options – trays & professionally assisted bleaching
Other options to same-day tooth bleaching include using trays daily or nightly over longer periods. These may be cheaper initially but require multiple visits and can cause sensitivity when used for weeks, especially if you have receding gums or many fillings.
The pictures below show Dr. Kilcoyne teaching dentists how to construct these trays and how to apply the gel correctly to get optimum results.
Professionally assisted tooth whitening may be started at the dentist’s and then completed at home under instructions, using the materials your dentist provides. Typically this involves around 2 weeks of wearing bleaching trays nightly, using the prescribed bleaching gel. Resistant discolouration may need many more days/weeks of tray bleaching before a satisfactory result. Everyone is different and unique.
How much improvement can I expect? Sensitivity & 24-hour advice
Everyone differs in how their teeth respond and in the causes of discolouration. With our same-day treatments, we often see an average of 8–9 shade improvement in brightness, with a small loss of a couple of shades over the first few weeks and gradual changes thereafter.
Tray-based whitening typically achieves several shade improvements (4–5 shades on a dental shade guide), with some gradual darkening over time. Usually, the darker the teeth are to begin with, the greater the scope for improvement. Some stains (such as greys) are more resistant and may need longer bleaching, combined systems or, in some cases, veneers or crowns.
Sensitivity and the first 24 hours after same-day whitening
Some sensitivity is normal – usually short-lived (seconds) and on/off. If anything goes beyond short sensitivity, take your usual painkillers; it is rare to need further attention. If at all concerned, contact the treating dentist directly for advice.
Diet-wise you need to be “good” for 24 hours. Basically, for just 24 hours avoid anything in your mouth that would stain a white shirt or blouse on contact. For example:
- If you are a red wine drinker, swap to white wine temporarily.
- If you must have tea or coffee, drink through a straw and rinse with water afterwards.
- Prepare simple, relatively stain-free meals (chicken, white rice, fish without dark skin, bananas, semolina, etc.).
Before your visit, have any curry, red wine, coffee/tea and then be ready to follow “white” diet advice for the rest of that day.

How does “professional” tooth whitening work?
Your dentist or specialist has professionally examined your teeth and mouth, considered your medical history and medications, and checked for
complications such as receding gums, leaking fillings or mouth ulcers that could leave lasting problems if whitening is done incorrectly.
You will then be given an overall plan so whitening won’t make existing or future crowns, veneers and fillings worse. You’ll be prescribed the best bleaching concentration and application for your needs and wishes, and carefully monitored during and after treatment.
Unfortunately, European legislation historically limited over-the-counter bleaching strengths, creating legal grey areas for dentists using higher concentrations for purely cosmetic purposes. There has been much debate in the UK about the legal position on tooth bleaching, and the General Dental Council (GDC) has tried to balance patient interests and law sensibly.
The law changed from 1st November 2012, confirming that tooth whitening is dentistry and that only dental professionals, following a proper examination, should prescribe whitening treatments. Further detail on this – including GDC guidance and EU law interpretation – follows in the next section (see below in the legal/FAQ part of the page).
Frequently asked questions about tooth whitening
Here are some of the common questions we are asked by both the public and fellow professionals when Dr. Kilcoyne lectures on tooth whitening. We’ve also added a few extra questions that patients often ask in clinic.
“Tooth whitening” has become a marketing phrase. Many products – including some toothpastes and even chewing gum – use the term to imply they actively whiten teeth significantly, when in reality they mainly reduce surface stains.
Over-the-counter products are much weaker than professional systems for safety reasons. Their packaging often gives big promises in large print, with careful disclaimers hidden in tiny print. Many are best described as “stain reducing” rather than true “tooth whitening”.
True whitening of tooth substance requires dentist-prescribed bleaching gels at an appropriate concentration, applied carefully and safely after a full examination.
We usually advise trying tooth bleaching first to avoid or reduce the need for drilling away healthy tooth for veneers or crowns. Tooth whitening can sometimes reduce the perceived need for extensive cosmetic dentistry or even some cosmetic facial procedures.
Crowns, veneers and dentures can be excellent options, especially if teeth are already heavily filled, worn or need reshaping / repositioning. But these treatments usually involve irreversible removal of tooth substance, which is why whitening is often considered first where possible.

No treatment gives permanent change forever – not even cosmetic surgery or facelifts. Whitening is no different. Teeth gradually darken again over time as we eat and drink coloured foods and drinks.
Bleaching does not lighten artificial dental work such as fillings, crowns, veneers or denture teeth, but used incorrectly bleaching materials can damage these. Only natural tooth substance lightens, which is vital in treatment planning.
Typically, after a successful whitening course, you may need a “top-up” after a few years, depending on lifestyle. People who smoke or consume large amounts of tea, coffee, red wine etc. tend to need repeat treatment sooner than those who limit such drinks and maintain excellent cleaning routines.
“Power Bleaching” usually refers to high-concentration hydrogen peroxide gels used in the surgery, sometimes with lights or “lasers”. Ironically, very high concentrations can cause too much dehydration of teeth – giving an initial “white” appearance that can reverse as soon as the teeth rehydrate when you drink.
In Dr. Kilcoyne’s opinion, one of the biggest myths in tooth whitening is that “the higher the %, the better the whitening.” This is simply wrong. Excess dehydration can actually block deeper whitening effects unless specific steps are taken, such as Dr. Kilcoyne’s rehydration formula.

Legal position on tooth bleaching in the UK & EU
In the early 2000s, European legislation about hydrogen peroxide products sold directly to the public was included in the EU “Cosmetic” directives. This limited non-dental products to very low concentrations and created a legal grey area for dentists using higher concentrations for purely cosmetic reasons.
For over a decade, UK dentists could theoretically be labelled “criminals” for using concentrations above 0.1% H2O2 for cosmetic whitening, even when acting in their patients’ best interests. The General Dental Council (GDC) recognised this ethical and moral dilemma and issued guidance that put patients’ interests first while the law caught up.
Fortunately, EU law was amended and in the UK the EU Cosmetics Directive changes took effect from 1st November 2012. In simple terms this confirmed that:
- Tooth whitening is dentistry
- Only a dental professional can examine you and prescribe tooth whitening treatments
- Only supplies of further gels between 0.1% and 6% hydrogen peroxide should be obtained via dental practices
There is still some debate about the precise interpretation of the law for wholly in-surgery treatments vs at-home “self-dosing” products, but broadly the intention has been to protect the public from unsafe self-use and non-dental providers.

Alternatives to tooth bleaching – and when they’re appropriate
Alternatives to professionally supervised bleaching gels (usually stronger than the 0.1% hydrogen peroxide cosmetic limit) are either:
- Very ineffective (most shop-bought “whitening” products in the UK)
- Potentially unsafe (e.g. sodium perborate, unregulated acids)
- More invasive – involving permanent removal of tooth substance
Some abrasive or acid-based systems (e.g. HCl + pumice) can remove surface staining but may roughen or thin enamel, leading to increased staining and sensitivity in the long term.
Restorative alternatives include various veneers and crowns. These can be an excellent choice if teeth are already heavily filled, broken, root-treated or need re-shaping and re-alignment as well as colour change. However, they are far more biologically “costly” than bleaching intact teeth.
The pictures above show how much tooth must be drilled even when a Specialist like Dr. Kilcoyne works very conservatively. Multiply this by 10 top and 10 bottom teeth, when whitening alone could have solved the colour issue, and you can see how tooth bleaching can save good tooth from being drilled away in many cases.

Professionally prescribed tooth whitening by dental professionals is proven to be safe and effective. It can:
- Save healthy tooth from unnecessary drilling for veneers or crowns

What are immediate dentures?
Put simply, they are temporary Dentures planned and made before extractions, so they can be fitted 'immediately' after any extractions but before the healing gum shrinkage has occurred, for social and basic eating convenience usually. The big plus is socially you aren't left weeks or months with no teeth as the gums heal & shrink after extractions - the negative is that they WILL need replacing early because your gums often shrink a lot and so the fit worsens quickly. That's why we call them temporary; they help you get through the biggest change period.
When might this be necessary?
When many of your teeth are loose or painful, they may be beyond saving. This condition is usually caused by advanced periodontal disease. Periodontal disease causes bone to be lost; if it's not caught in time, there's so little support for the teeth that they have to be removed.
Removing your teeth and replacing them with a denture may be the best way to eliminate any infection and restore the health of your mouth. When the extractions procedure and denture fitting are completed in one day, it's called an immediate denture. So what is typically involved?
To begin this process, your dentist first takes impressions of your mouth. From these impressions, precise working models of your mouth are made. It's from these models that the dentures are made. We'll work with you to select the best colour and shape for your new teeth. When your denture is ready, we'll extract your remaining teeth. You'll be thoroughly numbed before any teeth are removed, and should feel no pain. Naturally, things will feel very different. As soon as your teeth are out, your denture will immediately be placed in your mouth. For the first 24 hours, your new denture will feel tight because your gums are swollen. As your bone heals over the next weeks/month, your gums will shrink and your denture will feel loose. When this happens, we'll use a temporary lining material to tighten the fit. After this period of healing, when the
shape of your mouth has stabilised, we'll send your denture back to the lab and have it relined to improve it's fit.
RELINES ARE OFTEN REQUIRED AND ALWAYS ATTRACT ADDITIONAL CHARGES!
Some temporary problems are a normal part of adjusting to your new denture. At first your denture may tip when you chew. You may notice
increased salivation. The denture may seem bulky, and you might gag a little. Your tongue will feel crowded and you might have difficulty speaking normally. Like learning any new skill, at first it will feel awkward to eat with your new denture. But with time and practice, you'll improve, the only question is by how much. You may need another Denture sooner than predicted, if problems persist. Everyone is different.

What are dentures?
Dentures are removable appliances that replace missing teeth and surrounding tissues. They can restore appearance, support the lips and cheeks and improve chewing and speech when natural teeth have been lost.
What is the difference between full and partial dentures?
Full dentures replace all the teeth in one jaw, while partial dentures replace only some missing teeth and clip onto the remaining natural teeth or are supported by precision attachments.
How well will I be able to eat and speak with dentures?
There is usually an adjustment period while your muscles and tongue learn to control the dentures. Most patients gradually regain good function over a few weeks, especially if they start with softer foods and follow the advice given by the dental team.
How long does it take to get used to new dentures?

Advanced Partial, Part & complete Dentures at the Smile Specialist Centre
Specialist Dentist Dr.Kilcoyne provides many Advanced Precision and bespoke part dentures or complete dentures options, made specially just for you. With some teeth present, made healthy and useful, we can use precision attachments with little or no clasps showing, or non-metal dentures with tooth coloured (acetyl) resin clasps, or additional denture retention features; including using Dental Implants, mini Implants, immediate Dental Implants, Crowns, telescopic crowns, close fit Dentures, Magnets and other retentive features on teeth using RPI, Guide planes and other advanced precision systems too.
A detailed examination is always required for this detailed work, to improve your bite, fit, stability and cosmetics of your mouth overall.

How do I get a precision partial denture?
This needs careful planning by Dr.Kilcoyne prior to any provision and often utilises existing teeth/crowns to aid retention using precision attachments. A precision partial denture replaces missing teeth and restores your bite with a better fit and appearance than traditional partial dentures without attachments such as metal clasps.
Traditional dentures use metal clasps to hold it in place. Though these clasps do a fair job of holding a partial in place, they are visible and detract from your smile by looking unsightly or obvious even when you are just talking. See below how we can make a precision Denture which not only matches the subtly different colours in your Gums but matches existing teeth and crowns and even has tooth coloured clasps, if you look very closely you may just see them!

There are many other clever designs we can do too, with special new crowns and matching attachments built into the framework, newer designs allow the visible clasps to be eliminated, creating a much tighter fitting partial denture, with less metal showing too! However every Denture is hand-made and is unique to each patient, so Dr.Kilcoyne needs to do a thorough examination of your mouth, bite and Smile to help you get the best results for you, your smile, your function and of course your budget too.

The Diagram below shows a metal based Denture which is also less bulky than an all-plastic acrylic denture, especially behind the lower teeth, which then leaves more room for the tongue to move and speak etc. These kind of dentures are called Cobalt-Chrome part dentures, which you can ask your dentist about upgrading to from just the ordinary all plastic acrylic dentures.

As with all partial dentures, whatever kind or design you go for, good cleaning techniques are essential to keep the other teeth healthy, as they help hold in your denture and the danger is, you will neglect regular check-ups and professional cleanings, small problems will creep into the gums without you noticing and other teeth may get loser and be lost, resulting in the part Denture slowly growing into a FULL Denture and then what? A Full Denture only has shrinking gums to try and grip, so they don't perform as well as a part-denture that has healthy teeth to grip, so don't neglect those remaining teeth, keep them healthy through regular check-ups.
The Importance of good Aftercare and Maintenance, especially for part-dentures!
As well as regular check-ups to your Dentist to prevent problems (or at least catch them very early on), please follow your aftercare advice and don't neglect your Toothbrushing and Flossing Techniques that reduce Dental Plaque attack on your teeth, gums and dentistry work. Remember any remaining teeth help hold the denture in place - don't lose that grip!
