Please do complete the application form below, once you have read it all and are happy to commit yourself for the FULL year's training course, for usually one day per month (x12), starting at the Cedar Court Hotel, Bradford (near M606),UK. This limited, smaller group course will give the participants the individual attention and expert guidance to current best Practice protocols. I believe this course is unique in that it brings together
the most important aspects of Practice Management, Clinical Skills and Patient Satisfaction, in a time efficient, twelve-day course for the busy Practitioner, that proves itself month after month!
Relevant reading materials will be provided FREE on or prior to each day course (scientific papers, interesting articles, etc.) to help you get the most from each day. By the end of the course, each individual will also have a Practice Manual on disc/CD of useful protocols etc, which can easily be personalised. This will include a number of notices relevant to one's own Practice's needs. This is useful for both staff training/re-training and in complying with various new regulations and requirements, including Clinical Governance.
Year Course costs are £350 per day (if booked before 30th June 2012) inclusive (x12), less any discount (see below), to include course notes, materials, lunch, coffees and the references CD/Stick for those completing the course. Whilst you will be responsible for your own travel costs and accommodation as required, We will be pleased to assist with any individual arrangements as needed.

The aim of these 12 days is to enjoy the process of learning whilst making one's Practice environments better in many ways.
This 1 year supportive course tackles both the Clinical and Practice Management improvements together, so you can apply these sensibly over
a realistic timeframe.

To secure your place on this course, you are agreeing to pay the whole year's fees by one of the following options:

I wish to pay for 12 days in two instalments, one payment now, and remainder by the first day (20/09/12), for a 10% 4everlearning.com discount = two equal payments of £1890 (inclusive of £420 discount!)

YOU CAN ATTEND JUST SINGLE DAYS AT £499 inclusive per day - the £350 Rate is only for those showing commitment by paying a deposit in advance of the full 12 days Year Course by 30th June 2012 !
Thus those that sign up for the full year course are in effect getting several days FREE (even without the additional discounts) as well as the advantages of the Comprehensive Notes/CD/Memory Stick and applying their new found knowledge and returning for continuous support, development & feedback throughout the Peer-review year, benefiting everyone!
Cancellation and refund terms for whole or any part of this year's course fees due are:
More than one month's written notice received before the first course day = Refund of total fees paid.
Greater than two weeks written notice received before the first course day = 50% year's refund (50% of year fees due).
Less than two weeks written notice received before the first course day = No refund (the full year fees are still due).
(If you cannot attend ANY date/dates after starting, no refunds are possible, but your place is transferable to a named Colleague)
