Sometimes though there is no suitable alternative to extraction; your tooth may have to be removed because the decay doesn't leave enough sound tooth to be useful, even after root canal treatment and crowning, or the gum disease is so bad that loss of the underneath bone and gums means the tooth is very loose and cannot firm up after extensive or deep gum treatments, or trauma has resulted in fractured teeth or broken and cracked roots, requiring at least some extractions. But, sometimes a tooth or even many teeth can be saved, using some advanced Restorative and Cosmetic dental treatment alternatives too.
See below for just some examples where people had been advised that maybe tooth-loss and dentures was their only or likely alternative. Dr.Kilcoyne can't save every tooth of course, but even where he can't there is often an alternative to complete tooth loss, that would meet your needs in a way you'd be happy with!
Other possible alternatives to ALL teeth out and Dentures can include the following:
1. Root canal therapy to remove dead/dying or infected nerves, usually followed by placement of a crown.
2. Deep gum treatment for Gum Disease, to improve foundations around the tooth.
3. Surgery - usually bone grafting or root amputation/apicectomy to keep the good bit of tooth.
4. Delaying treatment - this is not recommended. The problem will only worsen over time, with further complications, bone loss and pain likely.
5. Advanced Crown or Bridgework to fix and replace teeth.
6. Dental Tooth Implants to replace a tooth that cannot be saved.

Root Canal treatment is probably the commonest alternative to losing the tooth, where there has been a lot of decay that has gone deeper and reached the nerve. If the bone around the tooth is infected, this condition will also worsen also but as long as the Gum and Bone support is reasonable, a root Canal treatment has an 85% chance of working. The sooner this is done though the better, otherwise the tooth could become unsavable if it all rots away.
However, when decay has progressed to the point where the tooth has to be removed, it is crucial to treat the problem as soon as possible. Otherwise, the infection will spread to other teeth, or worse yet, to the rest of your body. Jaw infections can even be life-threatening and still in the 21st Century, healthy adults die from blood-poisoning infections from tooth abscesses. Remember, it's often important to replace a tooth after it's been extracted. A vital missing tooth can set off a chain reaction in your mouth, resulting in many new problems, such as drifting teeth or over-eruption and occlusal or even TMJ problems. Losing a tooth is like losing 2 chewing teeth (the one opposite is useless), so more pressure may be put upon the remaining teeth, causing them overload or other complications too!
The Picture below shows teeth that are in a poor state, mainly through Diet that feeds the Bacteria with Sugar often and a lack of ToothBrushing the Teeth and Gums to remove the damaging Plaque layer of bacteria, which then invade the teeth and gums and rot and infect everything, as well as getting into your bloodstream. Thus if the patient isn't willing to improve their diet and cleaning habits, this will all just OCCUR AGAIN and the Teeth will be lost. However with deeper gum cleanings and treatment, fillings and possible Root Canal treatment if decay has gone deep near the nerve, even this mouth could have the Teeth saved IF the Dentist is free to do their Best and the Patient changes some bad habits and neglect too!
There are a number of Extraction alternatives where you may save a tooth from Extraction using more advanced options or you still have to lose say the worst tooth that if kept would compromise other teeth, but then the others can be saved by a variety of options - that's why you need a detailed Examination first so that you can be given an accurate assessment of which teeth are savable, the time and costs involved nd of course, how to prevent the same thing happening again in the future and losing teeth again later on.
Other possible Cosmetic Dentistry plans include bleaching ALL the natural teeth first that show in your smile. Although this won't lighten artificial materials such as Fillings, Crowns or Veneers, these can then be replaced (especially if old/discoloured) to match the "new" lighter bleached colour, so the whole smile improves and can dramatically improve the cosmetic aspects of your smile, at a low biological cost and can be combined with other dental treatments too!

Certainly in the USA, there is a much greater tendency to have teeth bleached as white as possible, with earlier orthodontic braces having expanded the arch Cosmetically so that there appears to be a great wall of even teeth when smiling. This though, breaks the rules for "naturally" beautiful smiles, following instead the "hollywood" rules for extreme fashion demands. This "Hollywood" influence is becoming more prominent now in the UK too, via film, fashion magazines, Celebrities etc.
Indeed, many American Film Stars have their teeth bleached very regularly, or even Veneered or if teeth are weak, then Cosmetic Crowns too!

Dental tooth Implants are an increasingly common choice, because they are the closest thing we have to replacing individual teeth with something fixed-in and very similar.
